Let’s Talk Therapy PLLC is committed to protecting client rights, maintaining the confidentiality of health information, and providing clients with information about accessing their records and submitting concerns or complaints.

This information is provided in accordance with applicable Texas law, including the Texas Medical Records Privacy Act.

Requesting Your Health Care Records

Clients may request access to or copies of their health care records maintained by Let’s Talk Therapy PLLC.

Please include enough information for us to identify the records being requested, such as:

  • Your full name

  • Date of birth

  • Dates of service, if known

  • The records you are requesting

  • Your preferred method of receiving the records

  • Your signature or other authorization, when required

Additional information may be requested to verify your identity and protect the confidentiality of your health information.

Requests will be processed in accordance with applicable state and federal law.

Licensing and Disciplinary Complaints

Clients have the right to contact the state licensing or regulatory authority responsible for the professional who provided their services.

Texas Behavioral Health Executive Council

The Texas Behavioral Health Executive Council (BHEC) oversees several behavioral health licensing boards in Texas, including the Texas State Board of Examiners of Professional Counselors.

Complaints against a professional regulated by BHEC must be submitted using the Council's approved complaint form.

If your provider is regulated by another Texas licensing agency, you may contact that agency directly regarding its complaint process.

Supervised Clinicians, Associates, and Interns

Let’s Talk Therapy PLLC may provide services through clinicians who are practicing under supervision, including associates, interns, practicum students, or other professionals completing supervised clinical training.

Clients receiving services from a supervised clinician will be informed of that clinician’s professional or training status and, when applicable, the identity and credentials of the clinician’s supervisor.

Questions, concerns, or complaints regarding services provided by a supervised clinician may be directed to Let’s Talk Therapy PLLC and/or to the supervising clinician identified in the client’s informed consent or professional disclosure documentation.

Clients also retain the right to contact the appropriate licensing, regulatory, educational, or credentialing authority when applicable. Contacting Let’s Talk Therapy PLLC or the supervising clinician is not required before submitting a complaint to an outside regulatory authority.

Medical Privacy and Consumer Complaints

If you believe that your protected health information has been improperly accessed, used, disclosed, or otherwise handled, you may file a complaint with an appropriate state or federal agency.

Concerns About Your Care

Clients are encouraged to communicate concerns regarding services directly with Let’s Talk Therapy PLLC when they feel comfortable doing so.

Contacting the practice is not required before filing a complaint with a licensing, regulatory, state, or federal agency, and doing so does not limit a client's right to contact an appropriate regulatory authority.

This notice is intended to provide clients with information regarding health care records, licensing authorities, supervised clinical services, and consumer complaint procedures.

I’d keep the supervisor’s actual name and credentials in each intern’s individual disclosure form rather than hard-code one supervisor into the practice-wide website, unless Let’s Talk Therapy wants a separate staff-specific complaint directory.
